This notice explains how medical information about you may be used and disclosed, and how you can access this information.

Our Privacy Duties
Trusted Nursing is committed to protecting the privacy and security of protected health information, often called PHI. This notice applies to health information we create, receive, maintain, or transmit when providing in-home nursing services, care coordination, White Card support, or related administrative services.
We are required by law to maintain the privacy and security of PHI, provide this notice of our legal duties and privacy practices, follow the terms of the notice currently in effect, and notify affected individuals after certain breaches of unsecured PHI.
How We May Use and Disclose Your Information
- Treatment. We may use or share PHI with nurses, physicians, care team members, pharmacies, laboratories, or other health care professionals involved in your care.
- Payment. We may use or share PHI to confirm eligibility, coordinate benefits, submit or support claims, obtain payment, and answer billing or coverage questions.
- Health care operations. We may use or share PHI for quality review, care planning, training, licensing, credentialing, auditing, compliance, business administration, and service improvement.
- Family members and caregivers. We may share information with a family member, caregiver, personal representative, or other person involved in your care or payment for care when you agree, when you do not object, or when permitted by law.
- Required or permitted by law. We may use or disclose PHI for public health activities, health oversight, reports of abuse or neglect, workers' compensation, law enforcement, court or administrative orders, serious threats to health or safety, and other purposes allowed or required by law.
When Written Authorization Is Needed
Uses and disclosures not described in this notice generally require your written authorization. We will request written authorization before using or disclosing PHI for most marketing purposes, before selling PHI, and before using psychotherapy notes if such notes ever apply to our services. You may revoke an authorization in writing, except to the extent we have already relied on it.
Your Rights
- Access. You may ask to inspect or receive a copy of your health information, subject to limited exceptions allowed by law.
- Amendment. You may ask us to correct health information you believe is incomplete or incorrect. We may deny the request in certain cases, but we will explain why in writing.
- Accounting of disclosures. You may ask for a list of certain disclosures of your PHI made during a period allowed by law.
- Restrictions. You may ask us to limit certain uses or disclosures. We are not always required to agree, except for certain disclosures to a health plan when you pay out of pocket in full and the law requires honoring the request.
- Confidential communications. You may ask us to contact you in a specific way or at a specific location, and we will accommodate reasonable requests.
- Paper or electronic copy. You may ask for a paper copy of this notice at any time, even if you agreed to receive it electronically.
- Personal representative. You may choose someone to act for you if that person has legal authority to make health care decisions for you.
- Complaints. You may file a complaint with Trusted Nursing or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
Website and Jotform Contact Intake
Trusted Nursing uses Jotform only for website contact intake. Please do not submit emergency requests through the website. If you believe you have a medical emergency, call 911.
When contact form information includes health information and is received in connection with care or requested services, Trusted Nursing treats it according to this notice. When Jotform is used to collect PHI, it is used through HIPAA-enabled Jotform services with appropriate safeguards and a business associate agreement.
Changes to This Notice
We may change this notice at any time. A revised notice may apply to information we already have and information we receive in the future. The current notice will be posted on this website with its effective date.
